Refer a Patient

Dr. Kushwaha welcomes referrals for the full spectrum of adult cardiology care, including assessment of patients with suspected or known cardiovascular disease, risk factor evaluation, and specialized echocardiographic imaging.

Patients are seen in person for initial consultation. In very select circumstances, virtual (video) consultation may be offered.

How to Refer

Referrals can be sent via the following methods. Please indicate on the referral if you would prefer that Dr. Kushwaha sees your patient, otherwise they will be triaged to the first available cardiologist:

Please include:

  • A completed referral form (or standard consult note)
  • Relevant clinical notes and investigations (e.g. ECG, labs, imaging)
  • Contact details for both referring physician and patient

Available Tests & Services

  • Consultation & follow-up care in general cardiology
  • Echocardiography (transthoracic and transesophageal echo, stress echo, contrast echo)
  • Holter monitor / ambulatory ECG review
  • Nuclear cardiology (stress testing (SPECT), pyrophosphate scans)
  • Ambulatory blood pressure monitors
  • Coronary angioplaty and other structural heart interventions, pacemakers/defibrillators and electrophysiology studies, cardiac surgery: Dr. Kushwaha does not perform these procedures, but has a network of world-class cardiologists and cardiac surgeons in Toronto whom he will refer you to.
